Both organs go into the Organless Corpse in the East Wing Examination Room, which reanimates and must be defeated before you can take the Level 2 ID Wristband.
Spoiler warning: this guide covers the Grace’s Care Center sequence in full. Placing both organs triggers an enemy encounter.
What you need before you start
The organ puzzle depends on items gathered earlier in the Care Center. Make sure you have these before heading for the organs:
- Level 1 ID Wristband: progress through the West Wing to obtain it.
- Corrosive: collect it from the Conference Room in the East Wing and use it on the padlock of the Lead Researcher’s Office puzzle box.
- Leatherbound Notebook: found inside the Lead Researcher’s Office puzzle box. It contains the file titled “Unlocking the Organ Transport Box,” which explains the box controls used below.
Where to find the Artificial Heart
The Heart is kept in Cold Storage, but the door is blocked by a mechanism you cannot solve by hand. You need a tool from the West Wing first.

Step 1: Take the Wrench from the Custodian’s Office
Go to the Custodian’s Office in the West Wing and pick up the Wrench beside the typewriter.
Step 2: Open the Cold Storage door
Return to the Cold Storage door. Its red wheel mechanism breaks if you try to operate it by hand, so use the Wrench on it instead. Once the door opens, pick up the Artificial Heart inside.

Where to find the Artificial Lungs
The Lungs are not lying in a room. They are inside the Organ Transport Box, which is locked and must be opened with its rear panel controls. Sources describe the box as sitting in the back of a van or truck in the Garage.
Opening the Organ Transport Box
- In the Garage, pick up the Organ Transport Box from the back of the van.
- Open your inventory, select the box, and inspect it. Turn it around until you can access the rear panel.
- Set the circular dial to Hand.
- Move the long slider to the middle position.
- Press buttons 2 and 5 down until they sit flush. Buttons 1, 3, and 4 stay raised.
- Turn the dial back to Auto.
The box opens and yields the Artificial Lungs. If the sequence does not respond, check that the slider is centred and that only buttons 2 and 5 are pressed before you return the dial to Auto.

Placing both organs and collecting the Level 2 Wristband
Once you hold both organs, the remaining step happens in the East Wing Examination Room. Clear or prepare for nearby enemies before you start, because the encounter begins as soon as the organs are placed.
- Return to the Organless Corpse in the East Wing Examination Room.
- Insert the Artificial Heart and the Artificial Lungs. According to KeenGamer’s guide, the order does not matter.
- The corpse reanimates. Defeat it.
- Take the Level 2 ID Wristband from the corpse.

Difficulty and the Examination Room safe
Difficulty can change how hard the nearby combat is. PowerPyx lists different safe codes for the Examination Room on Casual and Insanity. That safe is separate from the Organ Transport Box, so do not use its codes in the box sequence above.
