In Diablo 4 Season 7, Fugitive Heads are rare Headrotten boss variants found through the Headhunt activity—not items lying around the map. Complete Whisper bounties in a Headhunt zone to reveal Exposed Roots, then investigate them for a chance to spawn an Uprooted Cocoon and its boss. If the boss appears with a Fugitive Head attached, defeat it to claim the reclaimed Head and return it to the Coven near the Tree of Whispers.

This guide covers the Season of Witchcraft mechanic. Blizzard’s cited Season 7 sources do not establish whether it remains available in the live game in October 2026.

How to get a Fugitive Head in a Headhunt

  1. Enter a Headhunt zone. These seasonal zones contain Headrotten enemies and Whisper activities. Blizzard describes the activity in its Season of Witchcraft overview.
  2. Complete Whisper bounties inside the zone. Doing so reveals Exposed Roots. Bounties completed elsewhere are not the trigger described by Blizzard.
  3. Investigate the Exposed Roots. A root can rarely become an Uprooted Cocoon, which spawns a Headrotten boss.
  4. Look for the rare boss variant. The boss usually appears as a Headless Husk. Rarely, it appears with a Fugitive Head still attached; defeat that variant to obtain a reclaimed Head.
  5. Return the Head to the Coven. Bring it to the Coven near the Tree of Whispers to advance seasonal progression and receive Occult Gem-related rewards.

The chain is random at more than one point: an Exposed Root may not become a Cocoon, and a Cocoon’s boss is usually a Headless Husk rather than the Fugitive Head variant. Blizzard does not provide a numeric spawn or drop rate in its Season 7 PTR overview, so there is no official percentage or guaranteed number of Heads per hour.

What improves the chance of an encounter?

Blizzard says the chance of seeing a Headrotten possessed by a Fugitive Head increases with Torment Tier and when using a Draught of Whispers. The PTR overview describes these boosts qualitatively, not as a specific rate. Raising the tier or using the draught improves the odds; neither guarantees a Fugitive Head.

Can you find Fugitive Heads in Rootholds?

Rootholds are another Season 7 activity featuring Headrotten. Blizzard’s PTR overview says you enter them using Whisper Wood sigils, which can be found in Whisper Caches and Silent Chests. The official material does not show that Rootholds are a faster or more reliable way to obtain Fugitive Heads than the Headhunt chain, so the bounty-and-root route is the clearly described method.
