Spoilers for Red Dead Redemption 2: Thomas Downes is the character whose encounter with Arthur Morgan is understood to lead to Arthur’s tuberculosis. It happens during “Money Lending and Other Sins III,” when Arthur visits Downes to collect a debt for Leopold Strauss.
How does Arthur encounter Thomas Downes?
In “Money Lending and Other Sins III,” Arthur rides to Downes’s ranch to collect the money Downes owes Strauss. During their confrontation, Arthur takes the rake Downes is using to defend himself and knocks him down. Downes then coughs blood into Arthur’s face. His wife intervenes and says Thomas is unwell; Arthur later returns to camp and tells Strauss that Downes is not doing well.

Does the game confirm that Downes infects Arthur?
The scene strongly implies that this is how Arthur contracts TB, and Downes is the character associated with Arthur’s illness. More precisely, the encounter is the story’s implied source: the scene description establishes the blood-coughing moment, but it does not establish the biological mechanics of transmission or the exact moment infection occurs.
Where does the mission title come from?
The mission is titled “Money Lending and Other Sins III.” Rockstar’s support documentation also uses that title, though its entry concerns a gameplay issue rather than Arthur’s illness: Rockstar Support’s Red Dead Redemption 2 title update notes. The scene details are described in an iRedDead guide to “Money Lending and Other Sins III”.
