You can’t play an NTSC-region Nintendo 3DS game on an unmodified PAL 3DS. Nintendo region-locks the 3DS family, and changing the console’s country setting does not change its purchase region. The documented software workaround is custom firmware such as Luma3DS, which can patch out region checks; it is a modification, not an official setting, and does not guarantee every game or feature will work.
Why an NTSC 3DS game won’t run on a PAL console
Nintendo’s rule is straightforward: a 3DS bought in the PAL region plays software from that region. Nintendo identifies the Nintendo 3DS, 3DS XL, 2DS, New 3DS, New 3DS XL and New 2DS XL as region-locked systems. Its region explanation treats Europe as one region, so “PAL” here refers to Nintendo’s sales region—not a video-output setting you can switch.
See Nintendo UK’s answer about playing software bought outside PAL and Nintendo’s 3DS family region-lock explanation.
Changing the country setting does not remove the lock
The country/region option in System Settings changes the system’s country or region profile; it does not convert the console to another purchase region or make it accept out-of-region 3DS games. Nintendo says the setting cannot be changed to a country outside the region where the console was purchased. If a Nintendo Network ID is linked, its country must also match the system’s country. Nintendo’s country/region setting instructions are not a region-lock bypass.

Ways to play the game
Use a PAL release
If the game has a PAL-region 3DS release, that is the straightforward option for keeping the console unmodified. Check that the specific cartridge is a PAL release; Nintendo provides guidance on identifying games from other regions. The existence of a PAL version depends on the title.
Use custom firmware
Luma3DS’s loader documentation describes patching the HOME Menu to skip region checks so region-free games can launch from it. This is a software modification, not a menu setting or cartridge adapter. The documentation establishes that region-check bypassing is possible, but does not promise compatibility for every out-of-region game, online feature, update or service. See the Luma3DS loader documentation.

The documentation is not a complete current installation guide. It does not establish a universal setup procedure or title-by-title compatibility, so do not assume that bypassing the check guarantees the full experience you want.
Use a console from the game’s region
A system from the game’s region avoids the mismatch that blocks the cartridge on a stock PAL console. This is a hardware alternative, rather than a change to your PAL system; the cited sources do not compare console prices, service status or account-transfer implications.

What about Nintendo DS and DSi games?
Do not apply the 3DS-game rule indiscriminately to every older cartridge. Nintendo’s operations manual says 3DS and DSi software must match the system’s region. Nintendo UK separately warns that some Nintendo DSi games bought outside PAL may not play on a PAL 3DS. That qualification is about DSi software; it does not establish that all original Nintendo DS cartridges are region-locked in the same way. Consult Nintendo’s Nintendo 3DS Hardware Operations Manual and Nintendo UK’s DS-game compatibility answer.
